Transforming community services

Our plan to transform our community mental health services in Leeds has been discussed thoroughly with staff, service users and partner organisations, as well as commissioners and the health scrutiny committee and changes are now starting to be made. 

We have now moved in to the implementation phase of our project, with the new Community Service launched on Monday 25 June 2012.

Here are the key elements, which will enable us to work in this new way;

Single Point of Access

Our new single point of access to all our services launched on 25 June 2012, with one new phone number for all referrals to mainstream mental health services, making access to services simpler. Referrals can be made by:

Community Mental Health Teams

We will have three Community Mental Health Teams covering the city and aligned with Intensive Community Services will be introduced over the next month. They will be based in three localities which are aligned to local authority and District Nursing Team boundaries. These are:

Leeds East North East

Community Mental Health Team will transfer to Asket Croft (Seacroft) in spring 2013. From Monday 25 June the CMHT team for this locality will be located at Millside in Meanwood until spring 2013 when they will move to Asket Croft.

Leeds West North West 

Community Mental Health Team will be based at St Mary’s Hospital (Armley) from Monday 18 June 2012.

Leeds South 

Community Mental Health Team is now based at Aire Court (Middleton).

Intensive Community Services

The Intensive Community Service is now an ageless service providing a mix of home visiting and acute day-treatment. This service will bring together the functions which were provided by Acute Community Services (adults), Mental Health Intermediate Care Team (Older People) and the Home Based Treatment Team (adult).

The new Intensive Community Services will be closely integrated with the three community teams outlined above, based at the same locations, beginning from 28 May 2012. 

Leeds East North East

Intensive Community Service at St.Mary’s House, Chapeltown from Monday 28 May 2012. 

Leeds West North West 

Intensive Community Service will be based at St Mary’s Hospital, Armley from Monday 28 May 2012.

Leeds South 

Intensive Community Service will be based at Aire Court, Middleton from Monday 28 May 2012.

Memory Services

The Memory service will operate as a city wide service, based at St Mary’s Hospital, Armley, but with a presence in the locality team hubs at St Mary’s House, Chapeltown and Aire Court, Middleton.

Young People with Dementia Team and Care Homes Team

            Young People with Dementia Team and the Care Homes Team will be based at St Mary’s Hospital, Armley. We do not have a definite date for this move as yet, but you will still be able to contact the teams via the Single Point of Access.

Community Learning Disability Teams

There will be three teams which will operate the same boundaries as the three locality teams. Leeds East North East will remain at West Point until moving to Asket Croft in spring 2013. Leeds West North West will be located at St Mary’s Hospital and Leeds South team at Aire Court. As of Monday 3 September, the CLDT will join SPA, using the same process of making a referral.

Although a number of our teams have begun moving locations to form the three locality teams, this is to prepare for the 25 June. All our community services will not change the way they work until the Single Point of Access goes live on Monday 25 June 2012.
