The pharmacy department works alongside the medical and nursing teams to ensure that medicines are used safely and appropriately. We also ensure medicines are dispensed in a timely and efficient manner and that service users have the information and education that they need to feel comfortable taking their medicines.
We also provide dispensing and clinical services to all inpatient units, day hospitals and outpatients.
- Liase with the primary care team on admission and discharge to ensure seamless care
- Visit both inpatient and external wards/units regularly to check that medications prescribed to patients are appropriate and that there are no interactions between these medications or contraindications to the patient taking them
- Work with the medical team to advise on treatment options for patients as well as reviewing treatment histories for patients unresponsive to current treatment
- Advise on trust formulary, to ensure that treatments are effective and appropriate
- Audit prescribing to ensure it is of the highest standard
- Counsel patients on their medicines both as inpatients and when collecting outpatient prescriptions
- Run education groups for patients and carers, both at trust hospitals and external units
- Run training sessions for medical and nursing staff
- Each pharmacy operates a medication helpline where you can talk to pharmacy staff in strictest confidence about any medication related topic. The numbers are given below; lines are open 9am to 5pm, Monday to Friday
- Supply medications for inpatients, outpatients, external units, day hospitals and community psychiatric teams
- Provide stock medicines for the wards and outside units
Pharmacists provide the clinical services and detailed information for patients, carers and health professionals.
Technicians run the dispensing service, coordinate supply of medication as well as counselling patients on common side effects.
SATOs dispense and assemble medicines and order stock medication.
Inpatients
Inpatients will be able to speak with your ward pharmacist during one of the regular visits made to the ward.
