
The majority of specialist health interventions are delivered in the community by three multi-disciplinary Community Learning Disability Teams. The teams are made up of professionally qualified staff including; psychiatry, psychology, physiotherapy, dieticians, occupational therapy, speech and language therapy and learning disability nurses.
Assessment, treatment and care is provided within the framework of Care Programme Approach and the teams also provide specialist support and advice to a range of partner agencies.

We also have two Community Learning Disability Teams who across the whole of the city who specialise in working with service users with particular needs. these are;
The purpose of the team is to provide a service specifically for service users who have severe and complex challenging behaviours requiring highly skilled multi-disciplinary assessment and therapeutic interventions.
The purpose of the team is to provide a service specifically for Service Users with profound and multiple learning disabilities whose complex health needs are associated with extremely high levels of dependence and their requirements for multiple and complex interventions to maintain their health status.
Within this team there is also the Day Services Nursing Team. This team of registered learning disability nurses are based in four Local Authority Day Centres in Horsforth, West Ardsley, Rothwell and Seacroft as well as Vine Further Education Centre. The team provides direct nursing care and liaison with other professionals and agencies.
