Our Older People's Service provides a range of services to older people who have a mental health need and for all adults with a diagnosis of dementia. The services currently provided are;
The Community Mental Health Service provides assessment, treatment and support based on individual needs. They work in partnership with other health, voluntary, independent and social care services.
Day hospital services are provided within the community mental health units. Attendance at day hospital is arranged as part of a persons care package by the community mental health team. The service provides assessment, treatment and support to people with either mental health problems or dementia. People who attend this service have been assessed as requiring support in a mental health environment.
Respite services are offered over 24 hours to people whose needs cannot be met in a non- specialist environment. Respite is provided as part of a persons care package and is arranged by staff working within the community mental health team. This is a York based service.
The Memory Service provides specialist assessment, diagnosis, treatment and support for people experiencing memory problems.
The Younger People with Dementia Service provides assessment, diagnosis, treatment and support to younger people in Leeds with dementia and their families.
The Mental Health Intermediate Care and Rapid Response Service provides intensive support in their home over a short period of time for people experiencing a crisis with their mental health or dementia.
Mental Health Inpatient Services provide assessment, treatment and care to people experiencing a crisis with their mental health, including people who might pose a risk to themselves or others and whose care requires management in a hospital environment over 24 hours.
Dementia Inpatient Services provide assessment, treatment and care to people experiencing a crisis with their dementia, including people who might pose a risk to themselves or others and whose care requires management in a hospital environment over 24 hours.
Bootham Park Hospital provides a separate male only unit to York based males who require assessment, treatment and care in a specialist environment because aspects of their care may pose specific challenges to others.
The Liaison Mental Health Service is based within the Leeds Teaching Hospitals Trust (LTHT). The service offers advice, support and education to LTHT ward staff about mental health diagnosis, treatment, care and discharge.
